USA Hockey has created an electronic version of the consent to treat form with a digital signature.  This is a secure electronic signature.  If you prefer to print it out and send it in, that is an option for you.  NOTE:  Most people find it easier to fill it out electronically, but print, sign, and mail it.  Otherwise, follow the instructions below and email the completed form to the IYHA Registrar.

1. Open the USA Consent to Treat form by going to the Documents section or click HERE.  
2. Fill out form
3. Click on red signature arrow
   check box - create a self signed digital ID for use w/ acrobat (click next)
   check box – New PKCS #12 digital ID file (click next)
   fill out name & email address (click next)
   create a password, confirm password (click finish)
4. Sign document – reenter password (click sign)
5. Save as your child's first & last name
6. Send to Beth Soberg at  as an attachment
